Insurance billing is complex! In accordance with our contracts with insurance companies, we have to bill each visit according to the specific services provided at that visit. This means some visits (those which take more time or require more medical expertise) will cost more than others.
In addition, Dr. Gray incorporates psychotherapy into her sessions to allow for longer visits and promote ongoing mental wellness rather than just prescribing medication to treat acute symptoms. Different insurance companies (or even different plans within the same company) have different rules about how they reimburse combination medication and psychotherapy visits. This makes it difficult to predict in advance exactly how much each session is going to cost out-of-pocket, which can be challenging for patients who have chosen high deductible insurance plans.
We do collect the estimated patient responsibility at the time of visit. If there is a balance remaining after your claim is processed, we will charge the card on file in accordance with our patient policies.
Your insurance company is your best resource for questions you may have about your bill if you choose to use insurance. We can tell you what we bill, but what they choose to do with that is plan-dependent. You may contact us with specific questions about the CPT codes we most commonly use. Patients who are out-of-network or do not wish to use their health insurance may be eligible for prompt-pay discounts, please contact us for further information.
Dr. Gray is in-network with most Blue Cross/Blue Shield plans, Health Choice, United Healthcare, Cigna, and Healthcare Highways. For the most accurate information, fax us a copy of your insurance card (or you can upload it to the patient portal) so we can confirm whether or not we are in-network for your plan. If we are not in-network with your insurance plan, payment in full is required at time of service but we are happy to furnish you with an itemized superbill that you can submit to your insurance for out-of-network reimbursement.
All patients are required to maintain a credit card on file with the patient portal for session fees, copays, missed appointment fees, and other applicable fees as outlined in patient policies. We accept Visa, Mastercard, Discover, and American Express.
One of the more common questions we get from patients after they receive their explanation of benefits from their insurance company is "Why was I billed twice on the same day?" Dr. Gray typically incorporates psychotherapy into her sessions to support a holistic approach to mental wellness. Insurance companies require the psychotherapy portion to be billed as a separate "add-on" code in addition to the medication management portion of the visit. Consequently you will often see two or more codes on your explanation of benefits for one session date. Typical codes are explained in more detail below:
--One code (9920x or 9921x) is called an "E & M" code for "evaluation and management". It captures complexity and effort that is spent thinking medically--symptoms, side effects, dosage adjustment, medication choices, ordering or reviewing lab tests, my physical and neurological observations, focusing on other medical problems, etc. The value of ‘x’ will vary.
--The other code (9083x) refers to any talking psychotherapy done in the session, based on the time spent doing that (determining the value of ‘x’).
--Sometimes sessions may be entirely one kind of activity, not both, so those dates may have only a single code.
--You may see various other codes depending on services performed, please ask us if you have any additional questions.
